What Is Low-Dose Naltrexone?

Naltrexone is an FDA-approved medication used at standard doses (50 mg) to treat opioid and alcohol use disorders. At much lower doses — typically 1.5 mg to 4.5 mg — it has demonstrated a very different and potentially powerful effect: immune modulation and anti-inflammatory activity.

Low-Dose Naltrexone (LDN) is an off-label therapy that has been used by integrative medicine practitioners for decades. A growing body of research and a large community of patients report significant benefits for autoimmune conditions, chronic pain, inflammatory diseases, and mood disorders.

The Science

How LDN Works

Immune Modulation

At low doses, naltrexone briefly blocks opioid receptors, prompting the body to upregulate its own endorphin production. This rebound effect helps regulate immune function and reduce inflammatory signaling.

Glial Cell Regulation

LDN is thought to reduce the activation of microglia — the brain's immune cells — which play a key role in neuroinflammation and chronic pain. This may explain its benefit in neurological and pain conditions.

Anti-Inflammatory Effects

By modulating toll-like receptor 4 (TLR4), LDN can reduce pro-inflammatory cytokine production, offering a systemic anti-inflammatory effect without the side effects of traditional immunosuppressants.

Endorphin Upregulation

The temporary opioid blockade causes a compensatory increase in endorphin levels, which supports mood, immune balance, and overall well-being — contributing to LDN's benefits in mood disorders and fatigue.
