If You've Been Told Hormones Are "Dangerous"… Let's Talk
If you've ever been told hormone therapy is dangerous, causes cancer, or leads to blood clots, you're not alone.
I hear this all the time. And I understand why people are scared.
But here's what most people were never told:
"Not all hormone therapy is the same. And the type of hormone you use matters more than anything."
What Is HRT?
Hormone Replacement Therapy (HRT) is used to restore declining hormone levels, especially during perimenopause, menopause, and andropause. This includes hormones like:
- Estradiol
- Progesterone
- Testosterone
When these hormones drop, people experience fatigue, weight gain, brain fog, mood changes, poor sleep, low libido, and more. When done correctly, hormone therapy can be life-changing.
What Is Synthetic HRT?
Synthetic HRT uses hormones that are not structurally identical to what your body naturally produces. Common examples include:
- Progestins (synthetic progesterone-like compounds)
- Conjugated estrogens
- Certain oral estrogen formulations
These hormones are similar to human hormones, but not the same. And that difference matters.
Because when the body doesn't fully recognize a hormone, it can bind differently to receptors, trigger unintended effects, and create more side effects. This is where many of the risks people associate with hormone therapy originated.
The Study That Changed Everything — And Misled Millions
A huge amount of fear around hormone therapy came from the Women's Health Initiative (WHI) study published in 2002. This study is still influencing how providers counsel patients today.
Here's the problem:
- It primarily used synthetic hormones, not bioidentical hormones
- The average age of participants was older than the typical patient starting hormone therapy
- Many women already had existing risk factors
- The results were widely generalized to all forms of hormone therapy
The messaging that came out of this study led to widespread fear and a dramatic drop in hormone use. But over time, deeper analysis and follow-up data revealed that the risks were not as simple — or as universal — as they were originally presented.
Yet the damage was already done.
Let's Be Honest About the Risks
Older studies using synthetic hormones were associated with increased risks such as:
- Blood clots
- Stroke
- Breast cancer concerns in certain populations
But those findings were largely tied to the type of hormones used. They do not automatically apply to bioidentical hormone therapy.
What Is BHRT (Bioidentical Hormone Replacement Therapy)?
Bioidentical hormones are molecularly identical to the hormones your body naturally produces. That means:
- Estradiol is estradiol
- Progesterone is progesterone
- Testosterone is testosterone
Your body recognizes them. Your receptors know exactly what to do with them. This allows for more natural signaling, more predictable responses, better symptom improvement, and fewer unwanted side effects when properly managed.
Progesterone vs. Progestins: Not Even Close
Progesterone (bioidentical) and progestins (synthetic) are not the same thing.
Micronized progesterone supports sleep, mood, brain health, hormonal balance, and fertility.
Progestins, on the other hand, have been associated with mood disturbances, less favorable metabolic effects, and increased risk profiles in certain studies. Yet they are often grouped together, which has created a lot of confusion.
What Changed Recently
Here's something most people haven't heard yet:
In 2025, the FDA removed long-standing boxed warnings from certain hormone therapies, including estradiol and testosterone. This reflects what many clinicians have been recognizing for years: the conversation around hormone therapy safety has evolved.
But patients are still being counseled based on fear from decades-old messaging. That's why so many people are still hesitant — they were never given updated information.
My Approach to Estrogen
I prescribe oral estradiol. And I do this intentionally.
When used appropriately, estradiol is safe and plays an important role in supporting cardiovascular health, preventing osteoporosis, and even preventing dementia.
Estrogen is not just about symptom relief. It has protective effects on the heart, blood vessels, brain, and bones.
Avoiding estrogen out of fear — especially when that fear is based on outdated or misinterpreted data — can do more harm than good.
Why I Choose BHRT
I am very intentional about the type of hormones I use. I use bioidentical hormone replacement therapy because I want to work with your physiology, not against it.
My goal is to restore balance, improve symptoms, support long-term health, and minimize unnecessary risk. I am not interested in forcing the body to adapt to something foreign when better options exist.
The Bottom Line
If you've been told that all hormone therapy is dangerous, it's time to ask a better question:
What kind of hormones are we talking about?
Because synthetic hormones and bioidentical hormones are not the same. And treating them like they are has led to years of confusion, fear, and patients going without the care they actually need.
